Before submitting an appeal you must first complete the UnitedHealthcare Request for Reconsideration Form. If you have already completed this form and believe further consideration is warranted, submit your appeal in writing to the address on the back of the patient's ID card.

These vendors have agreed to abide by the terms of the statutes at issue with regard to the confidentiality of this information.
