As a UnitedHealthcare contracted facility, you may request that we reconsider your Hospital Comparison Program results. The purpose of reconsideration is to allow us to correct an error relating to the omission of data or attribution of results. UnitedHealthcare will review the reconsideration request (with appropriate documentation), we will not however alter the analytical methodology.
Quality of Care results will be reconsidered for the following reasons:
- A Quality result is reported for a condition or procedure erroneously attributed to a facility.
- A Structure and Process attribute (i.e. Leapfrog scores, accreditation status, teaching status, participation in CMS Hospital Compare) was omitted or erroneously reported, according to the dates below:
- Leapfrog scores (publicly reported): as of September 2006
- CMS Hospital Compare participation: as of September 2006 (reflecting core measures submitted for calendar year 2005).
- Accreditation and teaching status: as of August 2006.
Cost Estimate results will be reconsidered for the following reason:
- A Cost result is reported for a condition or procedure erroneously attributed to a facility.
All requests for reconsideration must include your rationale for reconsideration and are required to submit:
- Facility completed Reconsideration Documentation Form (linked at the bottom of this page).
All relevant information should be submitted via the Reconsideration Form. UnitedHealthcare will request copies of additional
documents as needed.

UnitedHealthcare will review your request and you will be notified in writing within 30 days regarding the final reconsideration decision.
