Herceptin® Notice - 7/18/2006
Herceptin will only be eligible for reimbursement as a covered service by submitting a pathology report indicating confirmation of HER2 gene amplification in the patient for whom this is prescribed. The pathology report will only be required on the initial claim and is not necessary for all subsequent claims.
