Dedicated Support for Your Treatment Plan

For a patient with cancer, one of the most important relationships they have is with you, their physician.  UnitedHealthcare's Cancer Support Program is designed to support this key relationship.  The program offers individualized, comprehensive coaching and education for patients diagnosed with cancer (all types of cancer, both adult and pediatric).  The focus is primarily on supporting cancer patients at high risk for complications and side effects associated with their cancer or cancer treatment that would benefit from the support of our experienced cancer nurse advocates.

Our goals for this program include:

  • Support and reinforce patient adherence to your treatment plan
  • Provide information to encourage patients' active participation in their care
  • Help reduce complications related to the cancer diagnosis and treatments through targeted and timely interventions
  • Ensure that patients communicate changes in health status to you before symptoms escalate
  • Ensure that patients understand when and how to take their medications and provide self care
  • Increase awareness and informed choices about hospice and palliative care at end of life
  • Assist in coordination of care and benefit issues

The role of UnitedHealthcare's Cancer Nurse Advocates:

Our cancer nurse advocates, specially trained nurses with cancer treatment experience and/or certification, will:

  • Support and reinforce your treatment plan, emphasizing symptom management
  • Collaborate with you to fill gaps in education, support and management of patients
  • Work with patients and their caregivers to support their clinical and psychosocial needs in collaboration with program social workers and other resources

UnitedHealthcare Members Eligible for UnitedHealthcare's Cancer Support Program:

  • The Cancer Support Program option is available through the member's benefit plan
  • Member has been diagnosed with cancer
  • Member is in active treatment or end-stage management

UnitedHealthcare's Cancer Support Program is currently available in the following areas:

  • Georgia
  • Cincinnati and Dayton - Ohio
  • North Carolina
  • South Carolina
  • Kansas City - Kansas
  • South Florida, Tampa - Florida
  • Pennsylvania
  • Albany, Buffalo/Rochester, Syracuse - New York
  • PacifiCare members in California and Texas
  • Program is not available for the following health plans:  Oxford Health, MAMSI, or UnitedHealthcare Plan of the River Valley, Inc. (formerly known as John Deere Health).

  • Physician Referral.  UnitedHealthcare members that meet our clinical criteria can be easily referred to the program:
  • For patients insured by UnitedHealthcare that have the Cancer Support Program available in their market, we ask that you complete a brief Cancer Patient Referral form and fax form to our attention.  We will send you a confirmation within two business days regarding the status of the referral.  You may designate a nurse or other clinician as a primary contact.
  • Patient Self-Referral
  • UnitedHealthcare identification through claims analysis
  • Referral from other care management program
