UnitedHealthcare is participating in a collaborative effort with GlaxoSmithKline (GSK), through which we have teamed to offer physicians in Florida, Georgia, Kansas, Ohio and Wisconsin, patient registry technology , free of charge*, to help manage thier patients with chronic disease. The BridgingCare Planner, a web-based patient registry, is designed to support improvement in health outcomes for patients with diabetes and cardiovascular disease. This important tool complies with laws and regulations regarding the privacy and confidentiality of patient information, including HIPAA. The BridgingCare Planner assists physicians in the delivery of evidence-based medicine by incorporating clinical guidelines at the point-of-care. Through the use of the BridgingCare Planner and an accompanying pen-to-paper Visit Planner, the physician can track a patient with chronic conditions, promote and support planned visits, and evaluate their patient population over time through outreach and reporting.
