Personal Health Summaries are valuable tools designed to help patients and their health care professionals keep track of personal health information, to help patients prepare for doctor’s visits or medical procedures, and to encourage greater involvement by patients in their health care. Personal Health Summaries include information received and maintained by UnitedHealthcare to pay for individuals’ health care and operate our business, including information about health conditions, medications, medical procedures and lab results. The information presented within a Personal Health Summary reflects only information available on the date accessed and may not represent a complete and accurate picture of health history. A Personal Health Summary does not contain certain types of information which require special privacy protections under federal laws governing alcohol and drug abuse information and state laws. These state laws regulate information such as HIV/AIDS, mental health, genetic tests, alcohol and drug abuse, sexually transmitted diseases and reproductive information.
