The National Committee for Quality Assurance (NCQA) Physician Recognition Program evaluates and recognizes physician performance as compared against evidence-based and expert physician derived measures in the following three areas:
Diabetes Physician Recognition Program (DPRP): Program criteria include achievement of clinical targets for:
- HbA1c
- blood pressure and lipid control
- eye and foot exams
- nephropathy screening
- smoking status and cessation advice

Heart/Stroke Recognition Program (HSRP): Program criteria include achievement of clinical targets for:
- blood pressure and lipid control
- use of aspirin or another antithrombotic
- smoking status and cessation advice

Physician Practice Connection (PPC): Recognition based on implementation of specific processes to reduce errors and increase quality for all patients, as well as demonstrating ability to manage care effectively in a complex environment.
Program criteria include:
- registries for identifying patient conditions
- reminder programs
- resources for patient education
- support and care management.

- Designation on consumer and health organization Internet sites such as the American Diabetes Association, American Heart Association, American Stroke Association, and the NCQA
- Recognition on the UnitedHealthcare Physician and Provider Directories.
- Complying with the required criteria that are increasingly being used as a basis for performance financial incentive programs such as the employer sponsored Bridges to Excellence® initiative.
